Platform consists of access to the SocialClimb web application for an unlimited number of users. The base platform allows access to reporting and insights. Some reports require additional products and/or EHR to populate some reports. Access to the Platform includes functionality to sync and manage all Google Business Profiles.

Platform includes 1 data connection to EHR / PM system utilizing available connention types. Thse may include direct API, FTP, or Manual File Upload. If additional connections are needed, they will be provided at a cost of $200 per month per connection.

Both Customer and SocialClimb agree to work in good faith to ensure that SocialClimb implementation and Customer PM/EHR integration or data file transfer configuration requirements are completed within a reasonable period, subject to third party cooperation. If data file transfer is required, then {Company Account Name} is responsible for preparation, manipulation, and delivery of the data file for upload into SocialClimb’s software. SocialClimb is not responsible for any third-party costs charged to Customer for connection, access, or delivery of data.

SocialClimb will aid Customer in claiming ownership of and optimizing the relevant Business Profiles at time of implementation. Continued monitoring and optimization of Business Profiles are the responsibility of the Customer.

A Provider is any individual who is either a physician or a health care provider who schedules patients directly and is eligible for Business Profile pages on the major listings providers(Google, Apple). A healthcare provider that cannot be directly scheduled is a Mid-Level.

Invitations to leave a review are sent on behalf of Providers or Locations only, when susbcribed to Reputation, respectively. Survey invitations may be sent for Providers, Locations, or Mid-levels, when subscribed to Surveys, respectively.

Customer may add quantities of items above during the term of this agreement by notifying [email protected] or their assigned CSM or Support Representative. These additions will be valid at the rates above and will be valid for the term of this agreement and any renewal terms. If a new product is requested, Customer will be required to sign a new Service Order. Any reduction in products may be done at renewal time and will require a new signed service order.

CAHPS – This is a specific type of survey that requires a subscription to the SocialClimb Platform.

Patient Feedback Surveys are generated on behalf of a provider or location. Each Provider or Location must sign up for Surveys.

ROI Tracking allows you to track calls and clicks to any Google Business Profile, website, self-scheduling system, or other marketing campaign.

Call Tracking monthly pricing is for the maximum minutes and phone numbers listed on an applicable Service Order. Additional minutes and/or phone numbers will be billed at the rate of $0.10 per minute and/or $2.50 per phone number per month.

Predictive credits may be used at any time during the term. If you run out of credits during the term and want to add more, you will be required to pay any remaining balance of credits already used and then may purchase additional credits.

Number or credits represent a single ourtreach via postcard. Included in the outreach is the ability to upload the predictive list to Meta and includes 15 impressions for each targeted individual.
